Question: What is the fee for renewal?

Answer: The fees vary by profession, date of renewal and may vary by type.

Question: I never received my renewal form in the mail.

Answer: Since 2007, an oversized, brightly colored notification postcard is mailed to each licensee, to the mailing address on file, at the beginning of their renewal cycle. You may renew online, download a renewal from our website, or call to have a renewal form mailed, regardless if you received the notification postcard. It is ultimately the responsibility of the licensee to renew their license by the expiration date.

Question: How do I renew online?

Answer: Visit our website at www.ksbha.org and click on the Online License Renewal link.

Question: I do not wish to renew online, how else can I renew my license?

Answer: You may renew by using our form.  To download the form, please visit our website at www.ksbha.org and click on Forms, then click on the appropriate renewal form for your profession. Or call our office at (785) 296-0934 to request to have a renewal form mailed to you.  Renewal forms must be mailed, faxed or e-mailed to our office to be processed.

Question: Can I fax or e-mail the renewal form?

Answer: Yes, you may fax your renewal form to (785) 296-0852.  You may attach a scan of the completed renewal form in an e-mail and send to Lori.Barnes@ks.gov or Ronda.Bohannon@ks.gov.

Question: What designation (type) should I renew my license?

Answer: Please click on the link  http://www.ksbha.org/departments/licensing/licensetypes/licensetypes.shtml   for an explanation of the type of licenses available for each profession.

Question: If I change my designation (type) with renewal, how difficult is it to change it later?

Answer: You would need to complete the Designation Change Form available on our website at www.ksbha.org, provide proof of liability insurance, compliance with the Health Care Stabilization Fund, continuing education hours and fee if applicable for your profession.

Question: How do I find my continuing education year?

Answer: From the home page on our website www.ksbha.org click on Verification and enter the required information.

Question: What does my continuing education year mean?

Answer: This is the year, you will verify you have completed the required continuing education hours.

Question: What practice address do I list since I work at multiple locations?

Answer: The online renewal allows you to enter up to three (3) locations.  If you are completing a renewal form, list your primary location on the form and the additional practice addresses may be submitted on a separate page.

Question: The online renewal asks how many patients I see on a weekly basis, but I don’t work in Kansas on a weekly basis, how do I answer?

Answer: You may estimate or average the number of patients you see per week. This is an optional field and will not affect the renewal from being completed if not answered.

Question: The online renewal system is stating my information is invalid, why?

Answer: Please verify that you have entered the full license number including your professional code (e.g. 04-11111). Please verify that you have entered your birth date as 8 digits with slashes (e.g. 01/01/1950) and enter the last 4 digits of your Social Security Number. Clicking the "Search" button should return your name and address information and allow you to create a username and password. If you are still receiving the error message after verifying your data entry, please contact our help line at (785) 296-4878.

Question: Can you tell me if my online renewal went through?

Answer: Your online renewal was completed if you received a confirmation by e-mail shortly after renewing.  If you did not receive the confirmation by e-mail your renewal was not complete. You may also verify from our website as of the next business day after you renewed online.  To verify from our website www.ksbha.org, click on Verification and enter the required information.

Question: I was almost done completing the online renewal and I received an error message, what does this mean?

Answer: Your online renewal was not completed, but the system usually keeps the information that was entered up to the point of failure. Please try to log back into the system in order to continue. If the problem persists, please contact our help line at (785) 296-4878. Or if the problem is with the payment portion of the process contact Kansas.gov at helpcenter@ink.org or call (785) 296-5059 or (800) 452-6727. Both Help Centers are available 8:00-5:00(CST) Monday through Friday for assistance.

Question: Is it possible to maintain an active designation (type) if I don’t practice in Kansas?

Answer: Yes, you will be required to obtain the continuing education hours and maintain liability insurance and the Kansas Health Care Stabilization Fund if applicable for your profession.

Question: I supervise nurse Practitioners; do I need to include them under those I supervise?

Answer: No, please notify us only of Physician Assistants, Athletic Trainers, and/or non-licensed persons performing radiological technology procedures that you supervise in the state of Kansas.

Question: I supervise physician assistants, athletic trainers, and/or non-licensed persons performing radiological technology procedures outside of Kansas, do I need to list them?

Answer: No, only include those that work in the state of Kansas on your renewal.

Question: I have an expired license(s) from other state(s), do I need to list those state(s)?

Answer: You must list all states that you have ever been licensed by, regardless of the status of the license.

Question: I have an inactive license, do I have to renew?

Answer: Yes, you must renew annually to maintain a current license.

Question: Can I change my designation (type) to active online?

Answer: No, you will need to complete the renewal form and provide proof of continuing education, liability insurance and compliance with the Kansas Healthcare Stabilization fund, if applicable.

Question: I have a healing arts active license and I perform locum tenens occasionally in Kansas, currently I do not have liability insurance and I am not compliant with the Kansas HealthCare Stabilization Fund, how do I renew?

Answer: To renew your license as active you must have proof of liability insurance and are compliant with the Kansas Healthcare Stabilization fund. You may change your license designation (type) to inactive or exempt with the renewal. Then once you are covered by liability insurance and the Kansas Healthcare Stabilization Fund complete a status change form to change your designation back to active.
