The Association of State Medical Board Executive Directors provides an online search tool for licensure verification. By using this service, you are able to search for license information on individuals licensed and regulated by the Kansas State Board of Healing Arts. All information available on this search engine is made available to provide immediate access for the convenience of interested persons. Written Verification Request (Official):

To obtain an offical written license verfication from the Kansas State Board of Healing Arts, you must submit a request to the Board using one of the two forms provided below.

Note: As of August 1, 2015, there is a $25.00 fee for written licensure verifications on the following professions: MD, DO, DC, DPM, PA, PT, PTA, ND, LRT, RT, OT, and OTA (please note there are two separate forms depending on the license type - see below)
